Coordinated Care for High-Risk Family and Trauma Needs
Training & PD
An intensive coordinated intervention for a client with complex trauma, family violence, and child protection involvement — resulting in reunification, voluntary care engagement, and a pathway back to study.

Background
The client presented with high and complex needs, requiring involvement from multiple services. There were significant safety concerns, including ongoing or historical family violence, child protection involvement, and complex trauma.
The client had a background of CPTSD and trauma, with shallow engagement and compliance across services. While multiple supports were involved, gaps remained in treatment coordination, risk management, and therapeutic support.
Our Approach
The intake appointment was prioritised to assess immediate risk, safety concerns, and service needs. Intensive case management was implemented, with SAMMs actively contacting and collaborating with mandated services to confirm engagement and clarify roles.
Weekly appointments and phone support were offered to meet the client's high and fluctuating needs. Where appropriate, outreach support was provided to reduce barriers to engagement.
A comprehensive safety plan was developed collaboratively between the clinician and client, with after-hours crisis resources provided. SAMMs worked to increase the client's self-awareness, engagement, and accountability while maintaining a trauma-informed and strengths-based approach.
